good morning to everyone:) . kindly help me and my friend in translation. when she came back here in japan, she brought along her 2 kids. her youngest was admitted in hokuen(tama ba spelling:confused: ). both of us are married to filipino guys, so the nihongo that we know are very limited. she ask my help for the questionnaire she received from them. we think its about the health condition of a child(hope we're right). sice i am a member here, i told her that if i have a problem regarding nihongo, i always rely on Timog Forum and then everything will be fine. everybody is willing to help. she told me that if everything is already settled, she will soon join TF. so again, i am asking your help to please translate this. it will be highly appreciated. thank you so much.

I usually keep my hands off stuff in an unfamiliar field/topic or that use too much unfamiliar/technical language. besides, I'm no mommy so I can't quite relate to a lot of child concerns :D

but it so happens I've done a similar translation before, so I managed to make heads and tails of the form you requested. (and I gather this is for personal consumption so it lessens the burden of responsibility should there be any lapses in translation:O)

anyway, I hope you can piece everything together as I've labeled the headings with the Japanese equivalents.
