Passport with valid entry to the Philippines
NBI Clearance or Police Clearance (to be authenticated by the Philippine Embassy and to be translate by english)
Medical Clearance (if abroad, must be authenticated by the Philippine Embassy and to be translate by english)
6 pieces 6" x 6" pictures
6 pieces 1" x 1" pictures
Bank Certification from an accredited bank of the Philippine Retirement Authority of bank deposit (US$ 50,000 for 50 years old & above, $75,000 for 35 to 49 years)
Processing fee: US$ 1,500

すでに観光ビザ以外の長期ビザをおもちの場 合は、申請前に観光ビザに切り替えておく必 要があります
3. 健康診断書
フィリピンで免許を持つ医師が発行したもの 、あるいは母国で受診し、英訳した上でフィ リピン大使館が認証したもの

4. フィリピン国家捜査局(NBI)発行の無犯 罪証明書
　　あるいは母国の最寄りの警察で発行され た無犯罪証明書でフィリピン大使館または領 事館が正式に認証したもの

5. 約1X1及び2X2インチの写 真6枚づつ

6. PRA指定銀行にて開設したUS＄定期預金 の預金証明書

a. 歳―歳: US$ 75,000
b. 歳以上: US$ 50,000
7. 手数料
a.　ビザ発行及びサービス料 US$ 1,500
