good day to everyone:) . kindly help me in translating this questionnaire that my nephew received from hoikuen. i had posted last july somewhat similar to this new thread. so in my own little way, i compared these questions from the previously translated questionnaire which hanikami san did for us:D . luckily, i found similar questions and will lessen the burden of Timog's translators. on the 1st file, who will fill up this questionnaire? parents or doctor? on the 2nd file, kindly translate the encircled numbers 1,2,3. thank you very much. the first form is supposed to be filled out by a doctor, except for the information inside the box which I've supplied below (file 0002).
following it are the three items you encircled in the second form(file 0003).

it's really hard to live in a foreign land especially for me who has a language barrier. but i'm trying my best to learn nihongo everyday and i guess, i am really lucky to have a mentor like you, who is always willing to help. i am really learning a lot from you and here in TF. thank you very much. wish i could return back a favor for you. anyway, right now the only thing that i can do best for you is to wish you good health and God bless you always.

type ko lang talaga ang nihongo (pero di ako type ng nihonjin, hehehe:p)
depende lang rin sa mood ko ang paggawa ko ng translation.
