Health Insurance card and Pension book with wrongly spelled name (Katakana)

I have just received my alien card, health insurance card, pension 'passbook', hanko, and have applied for a bank account.

My name, written in Katakana form, in the health insurance card and the pension 'passbook' was misspelled. Example, instead of ウィンストン, it became ウィルストン and a syllable (katakana character) of my Family name became ’メ’　instead of it being a 'マ’. Both items solely rely on the written Katakana name (no romaji form, for support).

Good thing, the alien card and bank account carry my name in 'normal' (romaji) form. It was our sachou who applied for our documents; thus, the wrong 'spellings'.

Are these katakana misspellings forgivable/acceptable, or should I demand for immediate replacement? I believe there is no certain rule for 'converting' foreign names into Katakana form, however my hanko (the one used for the bank) uses the 'correct' form.

never hurts to ask for a replacement... which, in my various experiences, the concerned offices have always done so willingly (along with profuse apologies for their error) :)

just point to them where they made the mistake and then make your request about having it changed.

pwede nyo pong papalitan yan para wala ka pong alalahanin if ever na misspelled man sya sa case ko cash card at credit card ko mali ang spelling ng katakana baliktad kaya pinapalitan ng asawa ko hintay lang kami ng 1 week tapos okey na libre sya :)

Problema lang siguro kasi baka tamarin sachou ko hehehe. At least nalaman ko na madali lang naman magpapalit. :)

punta po kayo kung saan nag apply ng insurance then show your alien certificate icocorrect po nila...tama po sila ... the rest hintay po naten ibang members:) good thing for me,thank you for sharing your experience :)

Dont worry dear,human mistake yan...at basta dala mo lahat ng identification mo,they can change that easily..
