gud am po mga ka tf...

help naman po sa mga nakakaalam kung ano po ang kailangan mga dalin documents sa phil.embassy pag nag pa change ng surname from married to single ...tsaka po kung may nakakaalam ng site ng embassy para po maka pag download ng application form,pasensya na po baka mag kagaya akong thread...salamat po sa mga makakatulong.

ito po ang mga requirements..

From Married Name to Single Name

1. Duly accomplished amendment form

2. Passport (Original and 1 photocopy each of 1st page, visa page & last page)

3. For divorced Filipinos:
Report of Divorce from the Philippine Embassy in Tokyo/Philippine Consulate General in Osaka (if divorced in Japan) or from the Embassy or Consulate in the country where the marriage took place (if divorced in another country) (Original and Photocopy)

For Filipinos with annulled marriage:
Amended Marriage Contract in security paper issued by the NSO authenticated by Authentication Division, DFA Manila (Original and Photocopy)
Certificate of Finality of Decision and Court Order/Decision authenticated by Authentication Division, DFA Manila (Original and Photocopy)

For widowed Filipinos:
If spouse is a Filipino: Death Certificate of Previous Spouse in security paper issued by the National Statistics Office (NSO) authenticated by Authentication Division, DFA Manila (Original and 1 Photocopy).
If spouse is a Japanese: Family Registry (Koseki Tohon) reflecting the Death of the husband/wife (Original and 1 Photocopy)
If spouse is a Foreign National other than Japanese:
Death Certificate (including English translation, if applicable) from the concerned Embassy (Original and 1 Photocopy)
4. If married in the Philippines:
Marriage Contract with previous spouse in security paper issued by the National Statistics Office (NSO), authenticated by Authentication Division, DFA Manila (Original and Photocopy)
If married in Japan or another country:
Report of Marriage issued by the Philippine Embassy in Tokyo/ Philippine Consulate General in Osaka or from the Embassy or Consulate in the country where the marriage took place. (Original and 1 Photocopy)

5. Birth Certificate in security paper issued by the National Statistics Office (NSO) (Original and 1 Photocopy)

NOTE: Amendment of passport can only be applied once to a current and valid passport. Further amendment will require application for a new passport.
Related Fees:
Amendment Fee: US$20
Verification of Koseki Tohon: US$25
Note: Fees are payable in US Dollar or it's equivalent in Japanese Yen. Please consult the Embassy for the current applicable rate.

sa mga nakakabasa po at may ganitong experienced ask ko lang po kung kailangan po ba munang ireport sa phil.embassy yung divorced nyo bago mag change from married name to single name? or okay lang kung dederetso na dun para mag change ng name?salamat po....

yep kelangan munang ireport ang divorce sa phil embassy para malaman nila ang status mo before you can change name into single name like what my friend did...

sa mga nakakabasa po at may ganitong experienced ask ko lang po kung kailangan po ba munang ireport sa phil.embassy yung divorced nyo bago mag change from married name to single name? or okay lang kung dederetso na dun para mag change ng name?salamat po....
oo..kailangan mo ereport sa phil.embassy ang divorce mo..pwede mo naman yata isabay ang ROD at amendment of passport sa pag apply...at kung nahirapan ka naman pumunta sa phil.embassy..pwede mo ipadala yan thru mail..

oo..kailangan mo ereport sa phil.embassy ang divorce mo..pwede mo naman yata isabay ang ROD at amendment of passport sa pag apply...at kung nahirapan ka naman pumunta sa phil.embassy..pwede mo ipadala yan thru mail..

thanks ulit sa inyo...siguro pede nga isabay kasi yung kasamahan ko sa work nung mag papakasal ulit sya sa bf nya punta syang phil.embassy to change her name dun na lang naireport yung divorced tapos sonomama na pinalitan nya na yung name nya.thanks po
